Transition and Young Adult Care pilots in Newcastle and Southampton: Early learning, practical insights and addressing inequalities

Clinical outcomes and engagement with care are shown to significantly decline between the ages of 16 and 20, however, there has been a lack of evidence on how best to improve outcomes and engagement. To address this, NHS England has funded a £2.5m Transition and Young Adult pilot programme, with 15 sites implementing a national service specification to drive improvement in clinical effectiveness and testing innovative ways to promote patient engagement. 

In this session, we will hear from the leads of two of these pilot site areas, Newcastle and Southampton, who will share practical insights about good practice and early successes based on their experiences of implementing their pilot young adult services, including what has worked well to engage key patient groups within the age cohort, and successes in addressing inequalities and promoting attendance.
