This short session aims to introduce you to how a person-centred approach can help people working in healthcare and people living with chronic conditions work together to health manage a chronic condition. Through the use of interactive activities, we will explore the complexities of behaviour change and managing a chronic condition, and the benefits of a person-centred approach during healthcare interactions with people living with a chronic condition.
