Sarah De Biase is Senior Programme Manager at West Yorkshire Integrated Care Board in the Long-term Conditions and Personalisation Function. Sarah programme manages the system diabetes programme along with the cardiovascular disease prevention programme and personalisation programme. Sarah is a physiotherapist and has significant clinical, programme management and leadership experience in health and care systems. Sarah is passionate about prevention, both prevention of long term conditions through healthy living and risk identification and management; and through enabling access to evidence based person-centred treatment and care which if engaged in can prevent progression of disease and multimorbidity such that people may live longer, healthier and happier lives.
